Full job description
Position: Social Worker
Shift: 8am-4:30pm
Status: Full-Time
As our long-term care social worker at Dove Healthcare - Osseo, you will be part of an interdisciplinary team providing case management for 40 residents. You will advocate for resident rights, coordinate services, and help meet the psycho-social needs of our residents.
You will be responsible for:

Evaluating resident psycho-social needs
-
Completing a comprehensive assessment and developing a plan of care
-
Exercising independent judgment / making sound decisions /exhibiting flexibility when needed

Working knowledge of Federal and State regulations
-
Ensuring and providing therapeutic interventions to assist residents in coping with their transition and adjustment to long-term care
Required Qualifications:

Bachelor's degree in social work or related human services field

Must hold current certification in the state of Wisconsin as a Certified Social Worker, and must maintain CEU's as per WI State Statute

*Full-time: 30+ hours per week
Part-time: 20-29 hours per week
