LPN (Licensed Practical Nurse) Lebanon Center for Rehabilitation and Healing
Lebanon, TN
Full-Time | Part-Time | Per Diem

Lebanon Center for Rehabilitation and Healing is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our growing clinical team. We are committed to providing exceptional care to our residents in a supportive and team-oriented rehabilitation and long-term care environment.
The ideal candidate will have strong clinical skills, a positive attitude, and a passion for delivering quality resident care.
Responsibilities

Administer medications and treatments as prescribed by physicians

Monitor residents’ conditions and report changes to the nursing team

Provide direct nursing care and support to residents

Maintain accurate and timely documentation in medical records

Collaborate with RNs, physicians, and interdisciplinary team members
-
Assist with admissions, discharges, and resident assessments
-
Ensure compliance with facility policies and healthcare regulations
-
Promote a safe, clean, and comfortable environment for residents
Qualifications
-
Current LPN license in the state of Tennessee

Previous experience in a skilled nursing, rehabilitation, or long-term care setting preferred

Strong clinical and communication skills

Ability to work effectively in a fast-paced environment
