How You’ll Make an Impact
• Assess & Plan
- Complete in-home RN assessments; craft individualized Care Plans with client/family approval.
• Coach & Supervise
- Orient caregivers to each Care Plan, perform field competency checks, and ensure ongoing compliance.
• Monitor & Document
- Track vitals, condition changes, and interventions in our HIPAA-secure EMR; escalate concerns promptly.
• Collaborate & Advocate
- Coordinate with physicians, therapists, and schedulers to keep care seamless and client-centered.
• Quality & Compliance
- Maintain audit-ready client files, collect initial service payments, and uphold state home-care regulations.
What You’ll Bring

Active Registered Nurse (RN) license in MD
-
Bachelor’s in Nursing or related human-services field – OR – 2+ years’ geriatric/home-care experience

Solid clinical assessment skills and a passion for problem-solving

CPR/First Aid certification and a valid driver’s license
-
Warm, professional communication that builds trust with families and field staff
