Responsibilities

  • Provide social work support, assessment, and resources to patients and families.

  • Help patients navigate healthcare, community resources, financial assistance, and other support services.

  • Collaborate with nurses, providers, case management, and the interdisciplinary care team.

  • Assist with discharge planning and transitions of care.

  • Advocate for patient needs while promoting dignity, independence, and well-being.

  • Provide emotional support, education, crisis intervention, and referrals as appropriate.

Qualifications

Education:

  • Bachelors Degree in Social Work from an accredited school of Social Work

Experience:

  • One year of clinical experience in health care preferred, but not required
