Job Description * Support our patients, families, and loved ones, as they go through their end of life journey * Perform a comprehensive assessment to establish goals for symptom management and end of life care with effective case management strategies * Be a member of an active interdisciplinary team that addresses patients’ needs holistically * Develop and maintain a therapeutic relationship by utilizing effective communication with patients and their families. * Utilize state-of-the-art technology to document skilled services provided. * Registered Nurse with Valid CT RN license * One year of RN experience; Homecare or Hospice experience preferred. * Valid driver’s license and reliable transportation required. * Computer proficiency required.
