Student Intern (Case Management-Community Mental Health and Wellness)

Job Title: Intern-Case Management

Program Title: Non-Medicaid and Health Home Case Management

Department Name: Community Mental Health and Wellness (CMHW)

The Community Mental Health and Wellness Division offers an array of services to meet the individual needs and strengths of youth and families. Our home and community-based case management program offers support to youth who have a mental health diagnosis.

Responsibilities

The intern will provide case management services and be required to complete documentation that complies with state, county, agency and program regulations.

Qualifications:

Bachelors level student in any human service related program with 2 years of experience relevant to their field of study.

Valid and clean NYS driver's license, along with a motor vehicle that is properly registered and insure with NYS.

Ability to do home and field visits required.

Schedule:

Days and times are flexible. Afternoon/early evenings or weekends when clients are available.
