Physical Therapist (PT)
Chestnut Park Rehabilitation & Nursing Center
Position Summary
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our team at Chestnut Park Rehabilitation & Nursing Center in Oneonta, NY. As a Physical Therapist at our facility, you will be responsible for providing high-quality, resident-centered physical therapy services to individuals requiring short-term rehabilitation and long-term care. You will evaluate residents, develop individualized treatment plans, and implement evidence-based interventions to restore function, improve mobility, reduce pain, and maximize independence and quality of life.
Employment Status
- Full-Time and Part-Time positions available
Compensation
- Hourly Rate: $45.00 – $55.00 per hour (Based on experience, qualifications, and licensure)

Essential Duties and Responsibilities

Perform comprehensive physical therapy evaluations upon admission and as needed
-
Develop and implement individualized treatment plans based on physician orders and clinical findings

Provide skilled interventions including therapeutic exercise, gait and transfer training, balance and fall prevention training, neuromuscular re-education, manual therapy techniques, and functional mobility training

Assess need for assistive devices and durable medical equipment (DME)
-
Monitor resident progress and adjust treatment plans accordingly
-
Complete timely and accurate documentation including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries in accordance with CMS, Medicare Part A and Part B guidelines

Participate in interdisciplinary care plan meetings and collaborate with nursing, social work, dietary, and activities staff

Provide education to residents and families regarding home exercise programs, safety, and discharge planning

Supervise Physical Therapist Assistants (PTAs) as applicable

Support facility QAPI initiatives and performance improvement efforts

Maintain infection control standards and adhere to facility safety policies
Qualifications
-
Graduate of an accredited Physical Therapy program
-
Current New York State Physical Therapist license in good standing
-
Current CPR certification
-
Experience in skilled nursing or geriatric rehabilitation preferred

Strong communication, organizational, and clinical decision-making skills

Ability to work collaboratively in a team-oriented environment
Physical Requirements

Ability to lift, transfer, and assist residents of varying mobility levels

Prolonged standing, walking, bending, and reaching

Ability to push and pull therapy equipment

Ability to respond quickly in emergency situations
