Join our DaVita team as a Social Worker (LCSW)! We are looking to hire a Part-Time resource who is able to support our patient population needs on-site at our Black Rock (Fairfield, CT) Dialysis Center ! This hire would work a 24-Hour week, the schedule is flexible however this hire responsible for patient entire census.

Requirements:

  • Master's Degree in Social Work (MSW) with a specialization in clinical practice from an accredited school of Social Work.

  • Meets all state required regulations to practice in the dialysis setting.

  • Obtained their Clinical License (e.g., LCSW, LICSW), meeting all licensure requirements for their state.

  • Proficient in all Experienced level Social Worker duties.

  • Intermediate computer skills.

  • Demonstrated knowledge of government and private insurance programs.

  • Excellent interpersonal and communication skills
