Job Description:
- Maintains a safe and clean working environment by complying with policies, procedures, rules, and regulations.
- Develops and maintains positive working relationships within the practice and other departments.
- Other duties as assigned. Implements and monitors patient care plans to ensure effective treatment.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as the primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders accurately and promptly. Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Leads and mentors staff to ensure optimal performance and competency in care delivery.
- Conducts holistic, individualized assessments of each patient considering age, physiological, and psychological status.
- Documents all phases of patient status while responsible for their care. Participates in the transfer of patient information to ensure continued quality care.
Education:
- Associate's of Nursing [Required]
- Bachelor's of Nursing [Preferred]
Licenses and Certifications:
- Registered Nurse (RN) [Required]
