Job Description

Conduct intake interviews with clients and caregivers, by phone and in person, to understand needs and identify appropriate services and referrals.

Complete comprehensive in-home assessments for frail and vulnerable adults, evaluating:

Living environment and safety

Functional and psychosocial status

Support systems, cultural considerations, and financial factors

Partner with clients and caregivers to link them to community-based services and resources that support independence and quality of life

Visit county senior centers and assess participants to determine appropriateness for center-based programming

Serve as a trusted point of contact for clients and families navigating complex systems
