Work Schedule: Days with Call Weekend and Holiday Rotation
Hours: 8:30am-4:30pm
Summary:
The Registered Nurse in the LIFE program can work in the home, clinic or the Day Center and promotes and maintains health of participants in the community through teaching, counseling, and appropriate preventive and rehabilitative services. Responsible for assessment of health needs and contributing to the plan of care, initial implementation of nursing care plans, and providing nursing care, as well as periodic re-evaluation of individual and family nursing needs.
Supervises home health aides in the home, clinic, and day center to ensure regulatory compliance and quality while maintaining an optimal level of independence for participants. Additionally, provides nursing support to the LIFE primary care staff. Demonstrates knowledge and skills necessary to assess, plan care for, and provide service to frail elderly participants according to assigned responsibilities and LIFE Program standards.

Educational Requirements:

Graduate from an accredited school of nursing, college or university required

Licensure and current registration as a Registered Professional Nurse in the State of New York

BSN preferred

BLS certification preferred

Knowledge of rules and regulations governing Licensed Home Health Agencies preferred
Experience Requirements:
-
One year of experience working with the frail elderly required

Minimum one (1) year relevant experience required

Home Care experience preferred

Acute medical surgical experience preferred

Experience with patient assessment instruments preferred
Knowledge, Skills, Ability:

Must possess a valid driver’s license and have a car available for work use
