Job Description:

Performs initial evaluations with patients and families. Assists with consultations and interventions for chronic disease management. Collaborates with physicians and healthcare team members. Develops and implements interdisciplinary treatment plans. Assists with advance care planning. Modifies care plans as needed. Facilitates decision-making regarding treatment. Works with healthcare professionals and community agencies. Communicates effectively with patients and families. Responds to patient needs. Maintains accurate patient records. Provides education related to palliative care. Other duties as assigned. Knowledge, Skills, and Abilities:
