Overview

SHIFT: 7 am to 7 pm one shift a week

Under the supervision of the Registered Nurse, provides direct patient care. Applies knowledge and skills by focusing on patient’s personal care, activity, nutrition, hydration, elimination, and prevention of health complications. Offers exceptional service and support to patients and their families during times of physical and cognitive ill health. Successful performance contributes to a holistic approach to the delivery of services, improved patient care, and a high level of satisfaction from patients and families. Acts in accordance with DNV Primary Stroke Center guidelines and educational requirements therein.

Responsibilities

  • Assists patients with activities of daily living.

  • Answers patients call lights.

  • Assists with simple procedures as defined in work area under the direction of the RN.

  • Takes/monitors and records vital signs, weights, and intake and output.

  • Transports patients, equipment, supplies, and specimens.

  • Labels all specimens in accordance with standards.

  • Assists in maintaining a clean, orderly and safe environment.

  • Uses technology to promptly and accurately document care provided.

  • Utilizes a holistic approach (considering physical, psych/social, spiritual, educational, safety, and related criteria) appropriate to the age of the patients served in the assigned service area.

  • Demonstrates ability to consistently meet the department work schedule.

  • Demonstrates initiative to improve quality and customer service.

  • Promptly completes documentation to assure an accurate legal record of patient care.

  • Reports in a timely manner, pertinent patient observations, significant incidents, or any changes in patient’s condition to the appropriate person.

  • Balances team and individual responsibilities, objective and open to other’s views, gives and welcomes feedback, contributes to positive team goals, and puts the success of the team above own interest.

  • Demonstrates effective communication with all members of the healthcare team.

  • Follows established, individualized plan of care and teaching plans based on identified patient needs, under the supervision of the Registered Nurse.

Qualifications

Education:

  • Graduate of high school or equivalent required.

  • Minimum of 75-hour nurse aide course required.

  • Validation Code of “Eligible” status by the Director Care Worker Registry preferred.

  • Basic computer/typing skills required.

  • Medical Terminology course strongly recommended. Experience:

  • Previous healthcare experience preferred. License(s)/Certification(s):

  • Must possess and maintain current Basic Life Support (BLS) and Mandatory Reporter certification or obtain certification within three months from date of hire.

  • Valid driver’s license when driving any vehicle for work-related reasons.
