Living by these virtues, we pride ourselves on exceptional service and the highest quality of care.The Advanced Heart Failure Nurse Navigator serves as a clinical expert and patient advocate for individuals with advanced heart failure and complex cardiovascular conditions. This role provides comprehensive care coordination, patient education, and ongoing support to patients throughout their treatment journey, ensuring adherence to evidence-based care plans and optimizing clinical outcomes.

As a Nurse Navigator, you will navigate the patient's care collaboratively across multiple disciplines, departments, and providers within the system to achieve seamless patient care. Includes the assessment and provision of all psychosocial resources necessary to support the patient and their family.

General Requirements:

General Requirements:

  • 2 years of RN experience required.
  • 3 years of experience in Oncology preferred

Education:

  • An associate or bachelor's degree from a nursing program accredited by ACEN or CCNE

License/Certification:

  • Must have an RN license in the state of Idaho issued prior to hire date.

  • Must provide licensure or proof of application in the State of Oregon within six months of starting.

  • Basic Life Support for Healthcare providers certification from either AHA, ARC or the Military Training Network is required at hire.

  • Oncology Nurse Certification or certification for defined service line and/or tumor site optional.
