Job Description:

Assists with coordination of diabetes education for all clinical staff, including physicians. Provides diabetes management and care orientation to new staff and students. Serves as a resource, mentor, and clinical expert on diabetes management for the hospital. Participates as a member of the Glycemic Management Ad Hoc Team. Assists with reviewing and disseminating diabetes program quality improvement and outcomes data. Maintains calendars, identifies conflicts, flexes schedules, and resolves issues as necessary. Provides diabetes education via telehealth if applicable Other duties as assigned. Provides inpatient consultation for diabetes management and evaluation in collaboration with primary care nurse. Collaborates with case management on continuity of care related to glycemic management. Documents assessment data and clinical recommendations grounded in evidence-based practice. Knowledge, Skills, and Abilities:

  • In-depth knowledge of current diabetes treatments and management; inpatient and outpatient populations. [Required]
  • Excellent customer service, organizational and problem-solving skills. [Required]
  • Proficiency in MS Office applications preferred: Word, Excel, PowerPoint, and Outlook [Required]
  • Works independently and demonstrates flexibility. [Required]
  • Demonstrates professional speaking, presentation and written skills. [Required]
  • Proven record of competence in staff and patient teaching. [Required]
  • Must be fluent in the English language to understand and communicate with patient population. [Required]

Education:

  • Associate [Required]
  • Bachelor's [Preferred]

Work Experience:

  • 1+ year of patient education [Required]

Licenses and Certifications:

  • Registered Nurse (RN) [Required]
  • Certified Diabetes Care and Education Specialist (CDCES) [Preferred]

Pay Range:

$68,132.50 - $119,520.35 Background Screening Requirement (Florida Law)
