We are seeking a compassionate, highly organized Registered Nurse (RN) to join our Palliative Care team serving Northern Wisconsin. This role supports both outpatient palliative services across multiple hospital sites and the central region, working closely with nurse practitioners, physicians, patients, families, and community partners to ensure coordinated, patient-centered care.
Position Overview
The Palliative Care RN plays a critical role in care coordination, clinical support, communication, scheduling, and tracking of palliative consults and follow-up services. This position requires excellent clinical judgment, strong communication skills, and comfort managing workflows across inpatient, outpatient, telehealth, and embedded clinic settings.
Qualifications

Active RN license in Wisconsin

Experience in palliative care, hospice, oncology, or complex care coordination preferred

Strong organizational, communication, and documentation skills

Ability to work collaboratively across multiple sites and disciplines

Comfort with EMR systems, Excel, telehealth platforms, and clinical workflows
