Full job description

Home Helpers Home Care of Manassas-Gainesville is seeking a compassionate, organized, and motivated Care Manager to join our growing team! If you're passionate about providing exceptional care, supporting caregivers, and making a difference in the lives of seniors and their families, we'd love to hear from you.

This is an excellent opportunity for someone who enjoys helping people, multitasking, building relationships, and making a positive impact in the healthcare field

Responsibilities:

  • Coordinate caregiver schedules and client assignments

  • Assist with onboarding and training new caregivers

  • Coordinate client admissions and nursing assessments

  • Develop and maintain care plans

  • Monitor WellSky documentation and compliance

  • Maintain accurate client and caregiver records

  • Communicate with caregivers, nurses, clients, and family members

  • Follow up on missed visits and scheduling changes

  • Ensure quality care and client satisfaction

  • Maintain HIPAA compliance

Qualifications:

  • High School Diploma or GED required

  • Associate's or Bachelor's degree in Healthcare Administration, Nursing, or related field preferred

  • Home care, healthcare, or care coordination experience preferred

  • Strong organizational and communication skills

  • Proficient with Microsoft Office and Outlook

  • Valid driver's license and reliable transportation
