Full job description

Become a part of our caring community Humana's Primary Care Organization is a provider of senior-focused, value-based care, with more than 340 centers across 15 states under the CenterWell and Conviva brands. This is a full-time, on-site, patient-facing role. You report to the Associate Medical Director and are responsible for both in-visit care and ongoing patient management.

About the Role

You will provide primary care for senior patients in a value-based care model focused on quality, prevention, and long-term outcomes.

You will care for patients with multiple chronic and complex conditions, manage ongoing care needs, and work with a versatile team.

What You'll Do

  • You will see 12-15 patients per day, allowing for longer, more in-depth visits

  • You will manage patients with complex and chronic conditions

  • You will complete accurate, same-day documentation to support patient care and outcomes

  • You will manage a daily clinical inbox (labs, messages, follow-ups)

  • You will coordinate care with specialists and care team members

  • You will apply Medicare risk adjustment principles through accurate condition documentation What You Can Expect

  • MD or DO from an accredited program

  • Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics

  • Active, unrestricted medical license (or ability to obtain) in South Carolina Preferred Qualifications

  • 2+ years of clinical experience with senior populations

  • Experience with Medicare Advantage or risk adjustment

  • Experience supervising Advanced Practice Providers (NPs/PAs)

  • Proficiency with electronic health records (EHR) and digital documentation. Compensation and Benefits You will receive a compensation package that may include:

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$255,000 - $298,500 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
