Job Description

Key Responsibilities

Provide nursing care, administer medications and treatments as prescribed.

Conduct routine assessments and monitor health conditions.

Accurately complete and maintain documentation, MARs, progress notes, and reports.

Support individuals in participating in daily routines and community activities.

Communicate health updates with families, team members, and medical providers.

Provide training and mentorship to Direct Support Professionals on health-related supports.

Promote dignity, respect, and self-direction for every individual.

Requirements

Active LPN license in the state of Minnesota.

Current CPR and First Aid certification.

Valid Minnesota driver’s license with acceptable driving record.

Must pass a MN DHS background study.

Physical ability to lift and transfer up to 60 pounds occasionally.

Experience with individuals with developmental disabilities preferred.
