Physical rehab is a 20-bed unit that helps patients recover from traumatic injuries, CVA'S, spinal injuries, amputations, and neurological conditions.
Performscarerequiringsubstantial specialized knowledge, judgement and nursing skills based upon the principles of psychological, biological,physicaland social sciences in the application of the nursing process. Utilizes high-level assessment skills to design and ensure multidisciplinary patient care to meet holistic needs, incorporating established practice guidelines and regulatory standards. Mustpossessabilityto supervise LPNs, CNAs, Techs, Unit Secretaries, and Clinical students. All employeeswill continuallymonitorand adhere to safety responsibilities and initiatives.
Description:
Complete and documentinitialand ongoing patient assessments.
Document changes in patient condition and critical events.
Participate in rounding and handoff reports.
Develop, implement, and update individualized plans ofcareeach shift.
Establish measurable patient goals and evaluate responses to interventions.
Provide patient and family education tailored to individual needs.
Initiate andparticipatein discharge planning using hospital and community resources.
Communicate patient status clearly with physicians, peers, and supervisors.
Documentcareaccurately according to policies and procedures.
Demonstrate knowledge of quality improvement practices to improve patient outcomes.
Provide carein accordance withhospital policies, procedures, and physician orders.
Perform nursing procedures safely and respond appropriately to emergencies.
Delegate tasks appropriately anddemonstratesound clinical judgment.
Use SBAR and ARCC communication to promote patient safety.
Organize workload effectively and prioritize patient care.
Facilitate teamwork, patient flow, and staff collaboration.
Act as a resource and role m odel for other staff members.
Participate in staff orientation, training, and professional development.
Maintain licensure, complete required education, andcomply withall regulations.
Attend requiredtrainingand report safety or compliance concerns promptly.
Requirements:
Current RN licensure and work permit in Kentucky.
Bachelor's or an associate degree in nursing from an accredited college/university.
RN Patient PT Deteriorations
Basic Life Support (BLS) certification isrequired. If not already certified,mustobtain BLS within30 daysof their hire date.
ACLS/PALS/NRP and successful completion of dysrhythmia class as required by department of assignment.
