Travel Physical Therapist | Home Health | $60/hr | Monday-Friday
Ready to make a real impact with patients in their homes while enjoying a consistent weekday schedule? We're seeking an experienced Physical Therapist for a 15-week travel contract serving patients throughout Southeastern Ohio.
? $60/hour
? Monday-Friday | 8:00 AM-4:30 PM
? 40 Hours Guaranteed
? Mileage Reimbursement Between Visits
? No Call & No Weekends
? Start Date: 8/17/2026
What You'll Do
-
Complete patient evaluations and reassessments
-
Perform OASIS documentation
-
Conduct Start of Care (SOC), ROC, Recertification, and Discharge visits
-
Develop and implement individualized treatment plans
-
Provide PTA supervision
-
Deliver exceptional patient care in a home health setting
Requirements
-
Active Ohio Physical Therapist License
-
Current CPR Certification
-
Valid Driver's License
-
Reliable Vehicle & Current Auto Insurance

Home Health experience required
-
HCHB/PointCare experience preferred
Coverage Area
Primarily serving Gallia and Jackson Counties , with occasional visits in surrounding areas.
