Key/Essential Functions & Responsibilities

  • Assists clients by assessing needs for available support services and facilitates access to program and State plan and other services, including vocational, medical, psychological and in-home services, as eligible and applicable for individual clients in accordance with the service definitions and parameters outlined in DDD policy.

  • Determines eligibility and secures Medicaid and DDD-funded services for clients ensuring that all available resources, including those not funded by the Division are considered and documents review of services.

  • Develops Individualized Service Plans (ISPs) for assigned participants based on assessment of needs and using person-centered planning process, with input from participants, their family and/or other designated team members, documenting services and supports needed.

  • Facilitates implementation of the ISP by obtaining services and supports, assists participants in identifying service providers and connects them to identified services.

  • Monitors provision of services, supports and participants’ needs, updating and maintaining ISPs, as applicable and ensuring delivery within allotted budget/coverage.

  • Conducts in-home/on-site visits to participants to assess quality of care and monitor services and supports received; appropriately documents findings in case notes and escalates concerns to supervisor.

  • Performs all other duties, as assigned.

Experienced Support Coordinators may be selected for assignment as Assistant Supervisor and will perform the following tasks, in addition to the above and maintaining their own caseload:

  • Assists in training new Support Coordinators and with their assigned caseloads through monthly shadowing, reviewing ISPs and providing feedback to Support Coordinator and Support Coordination Supervisor.

  • Serves as back-up to the Support Coordination Supervisor, providing coverage for support coordination and on-call coverage, as needed, in their absence.

  • Attends promotional events with or on behalf of Support Coordinator Supervisor to advertise available services, meet with provider agencies, etc.

Minimum Qualification Requirements including education, experience, licensure/certification, etc. and essential physical capabilities (e.g. lifting, assisting lifting, standing, etc.)

  • Bachelor’s Degree, preferably in social or human services field; and

  • One year of experience working with the adult I/DD population; and

  • Ability to respond to calls and programmatic needs beyond regular work hours/be on-call as needed

  • Must be at least (18) years of age

  • Must hold a valid Driver’s License with no more than 5 points

  • New Jersey State drug test screening is mandatory prior to employment and randomly/for cause, as applicable

  • Ability to travel very regularly (50 – 75%) throughout Somerset, Essex, Union, Sussex, Warren and/or Hudson counties.

  • Ability to work outside or regular business hours and/or on weekends, as requested by participants/their families

  • Excellent organization and communication skills, including above average writing skills

  • Above average computer skills, including ability to learn and work in NJ State mandated software program(s), Microsoft Office, Outlook

  • Ability to work independently

Support Coordination Assistant Supervisor must also meet the following additional requirements:

  • Two (2) or more years’ experience in a Support Coordinator role in the I/DD field

Preferred Qualification Requirements (desired requirements beyond MQRs above)

  • At least one (1) year of experience working in case management
