CHMC- Case Management– RN Case Manager – Per Diem- Day Shift– Req 9664
Pay Range: $37.63-$49.86*
We are seeking a highly organized and clinical RN Case Manager to oversee the care of complex patients. In this role, you will act as a central coordinator, negotiating and managing resources to ensure high-quality, cost-effective outcomes from admission through discharge.
Key Responsibilities

Care Coordination: Manage complex patient cases by collaborating with interdisciplinary teams both inside and outside the organization.

Strategic Planning: Merge clinical needs with financial processes, interpreting Medicare, Medicaid, and private insurance benefits to facilitate care.

Quality Improvement: Participate in performance evaluation processes to enhance patient care across the entire continuum.

Documentation & Compliance: Maintain accurate records, ensure patient confidentiality (HIPAA), and adhere to all regulatory and safety standards.

Advocacy: Use evidence-based best practices and disease management knowledge to guide treatment regimens and patient safety.
Qualifications & Requirements

Licensure: Current New York State RN License and registration.

Experience: Minimum of two (2) years of nursing experience.

Certifications: Current BLS certification.

Technical Skills: Basic computer proficiency and the ability to navigate complex medical/financial data.

Physical: Ability to stand, bend, and walk for extended periods.
