Full job description

Location: Essex County, NJ

Type: Full-Time, every other week 4:30pm - 8:30am, including every other weekend

Serenity Hospice provides caring and compassionate hospice care services to patients and families throughout New Jersey. Our mission is to alleviate symptoms, foster a state of calmness and harmony, and encourage an appreciation of family, friends, and life experiences.

Serenity Hospice is looking to add a dedicated and compassionate HOSPITAL ON CALL RN to provide exceptional care, comfort, and support for our patients. This position will cover hospital locations in Essex County and support the Northern Hospital Division team during on-call hours.

Responsibilities:

  • Complete evaluations, admissions, and discharges within our Northern Hospital Division as needed.

  • Medicare-eligibility review for new referrals and determining if patients meet criteria for GIP-level hospice.

  • Follow up on current GIP hospice patients to ensure needs are met during on call hours.

  • Reinforce hospice recommendations made by RN Case Managers and make adjustments as needed.

  • Collaborate with doctors, nurses, social workers, patients, families, and our hospice staff to promote exceptional end-of-life care.

  • Be available to answer any questions and provide education as needed regarding hospice services and philosophy.

Requirements:

  • Current NJ RN License in good standing

  • Strong communication and interpersonal skills

  • Hospital experience preferred but not required

  • Hospice experience preferred but not required

  • Willing to travel to assigned hospitals with reliable transportation and liability insurance
