The Clinical Pharmacist – Nursing Unit Based provides comprehensive clinical pharmacy services to support safe, effective, and evidence-based medication therapy for a general inpatient population. This role combines direct patient care with operational pharmacy responsibilities through a rotating practice model. During clinical rotations, caregivers perform patient profile reviews, optimize medication therapy, provide pharmacotherapy consults, educate patients, participate in interdisciplinary rounds, and precept pharmacy residents and students. Operational rotations focus on medication order verification, renal dose monitoring, and clinical consult support to ensure efficient pharmacy workflow. By collaborating closely with physicians, nurses, and other healthcare professionals, this pharmacist plays a key role in improving medication safety, optimizing therapeutic outcomes, and enhancing the overall quality of patient care.

A caregiver in this position will work a full-time schedule rotating between day and evening shifts. Day shifts include 7:00am to 3:30pm, 8:00am to 4:30pm, or a flexible start time between 7:00am and 9:00am, while the evening rotation consists of 3:00pm to 11:30pm for one week at a time. Caregivers participate in an every fourth weekend rotation, alternating between first- and second-shift weekends, and work two holidays per year (one major and one minor). Occasional overnight coverage, typically 3–4 consecutive nights approximately once per year, may be required to provide PTO coverage.

A caregiver who excels in this role will:

  • Provide safe, efficient, patient-centered, evidence-based medication services.

  • Verify medication orders and/or dispenses medications accurately according to state and federal regulations.

  • Document services in a complete, accurate and timely manner.

  • Actively contribute to medication safety initiatives.

  • Ensure all pharmacy operations are in compliance with national accreditation standards.

  • Directs staff appropriately to provide safe and efficient care and maintain professional working relationships with all caregivers.

  • Educate and train students, residents, other caregivers and/or patients.

Minimum qualifications for the ideal future caregiver include:

  • PharmD or Bachelor’s of Science in Pharmacy from an accredited college of Pharmacy

  • Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire

  • Completion of a Post-Graduate Year 1 Residency (PGY1), fellowship or relevant post-graduate pharmacy experience

Preferred qualifications for the ideal future caregiver include:

  • Board Certification

Physical Requirements :

  • Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time.
  • Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.
  • Position often includes reaching, stretching, stooping, climbing steps and sitting.
  • Must have normal or corrected vision.
  • Must have ability to clearly communicate verbally by phone or in person

Personal Protective Equipment :

  • Follows standard precautions using personal protective equipment as required.
