Most new graduate PAs and NPs who want to go into orthopedics face the same impossible catch-22: you need experience to get the job, but you need the job to get the experience. The specialty you actually want keeps telling you to come back when you've already done it somewhere else.

We built the program that fixes that.

The Genesis Orthopedic APP Clinical Fellowship is a 12-month, full-time, paid training year designed from scratch for new graduates who are serious about orthopedics and ready to do the work to get there. No prior ortho experience required. What we need is the aptitude, the drive, and the willingness to be coached hard by people who know this specialty inside and out.

By the end, you'll be ready to practice independently. And you'll already have the job.

The Opportunity

This isn't an observation year. It's not shadowing with a stipend.

From day one, you're embedded across the full continuum of orthopedic care — clinic, hospital, and OR — with responsibility that grows as your competency does. You'll have structured mentorship from Genesis surgeons and senior APPs, progressive autonomy as you earn it, and a clear runway to a permanent role within the practice when the year is done.

Twelve months. Real patients. Real procedures. Real orthopedics.

What You'll Build Competency in

Clinic

  • Independent orthopedic evaluations and treatment planning

  • Fracture and acute injury management

  • Post-op visits and wound care

  • Injections, splinting, casting, and in-office procedures

  • Imaging review and clinical decision-making

Hospital

  • Daily inpatient rounding

  • ED and floor consults

  • Orders, documentation, and discharge planning

  • Serving as the liaison between surgeons and hospital teams

Operating Room

  • Surgical first assist

  • Pre- and post-operative care coordination

  • Progressive OR responsibility as skills develop

This is the full spectrum. Most APPs take years to accumulate this exposure. You'll get it in one.

Who This is For

You just graduated — or you're about to. You're BC or board-eligible. You're licensed in Illinois or getting there. And you've known for a while that orthopedics is where you want to be.

You're coachable. This year will move fast and push hard. The APPs who get the most out of it are the ones who stay hungry, ask good questions, and don't confuse confidence with competence.

You're comfortable with complexity. Clinic to hospital to OR, sometimes in the same day. Multiple sites, rotating schedules, real call coverage. If that sounds like a lot — it is. If it sounds like exactly what you've been waiting for — keep reading.

You care about who you're treating. Genesis serves Medicaid patients, underinsured patients, and patients who had no real options before they found us. The clinical training here is exceptional. So is the reason it matters.

The Structure

  • Duration: 12 months, full-time

  • Schedule: Monday–Friday with rotating weekend call coverage (mentorship support on call)

  • Rotations: Clinic, hospital, and OR days across multiple sites

  • Locations: Chicago/Near West (Oak Park, Humboldt Park) or Western Suburbs (St. Charles, Aurora, Glen Ellyn)

  • Completion: Permanent Orthopedic APP role + salary increase upon successful graduation
