The Primary Care Provider will carry their own patient panel and provide quality, patient-centered care among a collaborative team. Role: Open to Nurse Practitioners and Physician Assistants Location: Plainville, CT Setting: 100% Outpatient Primary Care – Adult Medicine (18 ) Schedule: Full-time, weekdays only.
