Wider Circle is looking for a Care Navigator to join our team in Care Delivery Operations. In this entry-level role, you'll talk with patients by phone (and sometimes in person) to complete health assessments and intake appointments. You'll build quick trust with patients, ask about things that affect their health — like food, transportation, and housing — and keep clear, accurate notes on every call. This role is a strong first step toward becoming a Care Advocate, with a clear path to grow into that position.

Responsibilities

Run Health Screenings & Intake Calls: Call patients to complete health screenings and intake appointments using a standard set of questions. Build quick rapport so patients feel comfortable sharing accurate information during each 15–20 minute call.

Schedule & Follow Through: Book intake appointments directly in Healthie (no separate booking link needed). If a patient can't complete the intake on the spot, schedule it for yourself rather than passing them to someone else — this keeps the relationship consistent. Follow up to confirm appointments and keep show rates high.

Connect Patients to Resources: Ask open-ended questions to understand each patient's needs. After a completed intake, schedule the right next step — a visit with their doctor or a Community Health Worker.

Keep Accurate Records: Enter patient information into our records system in real time during each call. Review your notes for accuracy and finish all documentation before the end of your shift. Follow all privacy rules (HIPAA) and get formal patient consent for every call.

Meet Your Goals: Manage your daily schedule and consistently hit your targets, including a show rate of 60% or higher for scheduled intakes. Space out your appointments so you have time to review your notes carefully.

Requirements

Education: High school diploma or equivalent required. A degree in Human Services, Public Health, or Behavioral Health is a plus.

Bilingual: Fluency in English and Spanish (or another language) is strongly preferred.

Experience: Experience in healthcare, social services, care platforms like Healthie, or community outreach is helpful but not required. You should be comfortable talking with a wide range of patients by phone. No prior intake experience needed — we provide full training.

Comfortable with Technology: You should be comfortable using web and app-based systems, and able to adjust quickly in a fast-changing environment.

Works Well Independently: You can manage your own tasks, keep your documentation organized, and meet your goals while working remotely with limited supervision.

Willing to Train: You'll complete a full training program, including guided call practice, documentation training, and role-playing exercises. You'll need to score 80% or higher on the final training assessment.

Background Check: You'll need to pass a drug test and background check before starting.

To do this job well, Care Navigators need to be able to:

Sit, stand, and walk regularly to talk with members and manage day-to-day tasks.

See and hear well enough to talk with members by phone and in person, and answer detailed questions.
