Job Description

Job Summary:

This position will focus on provision of person-centered services and comprehensive care management including planning and coordination of all physical, behavioral and support services to increase independent living skills and positive community involvement for enrolled participants.

Essential Functions:

Comprehensive Care Management, Community Support Services and Coordination: Provides comprehensive care management and planning and coordination of all physical, behavioral, and support services for identified members as evidenced by:

Assisting in the development of and functioning as the coordinator of the comprehensive treatment/service plan in a manner that is person-centered and meets both agency and regulatory expectations.

Identifying and coordinating services and supports for assigned individuals as indicated by treatment/service plan goals.

Ensuring members and their identified supports have access to medical, behavioral health, pharmacology, and age appropriate resiliency and recovery support services.

Delivering health education specific to a member’s chronic conditions.

Coordinating primary, specialty and transitional health care from ED, hospitals and psychiatric residential treatment facilities.

Providing individual interventions to develop or enhance an member’s ability to make informed independent choices and increase independent functioning in the community.

Assisting in development of assigned members’ safety/crisis and self-management plans.

Demonstrating the ability to recognize and assess suicide potential and risk, physical health risk and provide appropriate responses in emergency/crisis situation.

Service Team Approach: Participates as an active treatment team member serving individuals with behavioral and/or physical health issues as evidenced by:

Demonstrating the ability to recognize and identify symptoms of mental illness and substance use.

Demonstrating the ability to identify issues/symptoms associated with physical health issues and chronic medical conditions.

Participating as a member of a multi-disciplinary team approach to treatment and recovery.

Coordinating with both internal and external providers regarding service provision.

Performs appropriate core services of the Health Home.

Outreach, Engagement, Follow-Up: Provides comprehensive outreach, engagement, and follow up to assigned individuals as evidenced by:

Providing comprehensive outreach and follow up to all assigned individuals including contact attempts over the phone, in-person, in the home etc.

Demonstrating the ability to engage and develop rapport with individuals presenting with a variety of symptoms/presentations.

Cultural Competence and Diversity: Provides culturally appropriate services to diverse populations as evidenced by:

Providing services that are person-centered.

Providing services that are culturally relevant.

Providing a non-judgmental, open, and welcoming approach to diverse populations.

Documentation: Completes documentation that meets identified expectations as evidenced by:

Documenting all services provided within expected time frames.

Documenting all services provided in a manner that meets service definitions, regulatory standards, and agency expectations.

Documenting all services provided in a manner that is person-centered and meets quality improvement goals.

Efficiency and Productivity: Consistently meeting established productivity standards as evidenced by:

Demonstrating excellent organizational skills.
