Description

Case Manager II - ATCM Deaf and Hard of Hearing Services (ASL Fluency)

People Incorporated is seeking Case Manager II's to join our ATCM Deaf and Hard of Hearing Services (DHHS Team) to work with clients who are diagnosed with chronic mental illness. We support people in connecting with resources to live as independently as possible. We help them develop self-awareness to make positive plans for the future through frequent meetings in the community. Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules.

Hours: Full - Time, Monday - Friday, business hours

Location: Minnesota l Hybrid

Range: $24.25 - $25.75 Hourly, based on qualifications

Hiring Bonus: $1,000 to external candidates

Main Job Duties:

  • Must be able to have onsite presence

  • Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.

  • Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.

  • Assessing client needs and ensuring continuity of care.

  • Making ongoing referrals to resources based on assessment and current client need.

  • Maintaining necessary daily records for continuity of service and reports to funding agencies.

  • Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.

  • Attending client monthly meetings to monitor services.

Qualifications

To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. These essential duties include:

  • Passing all background and requirements for facility and position; and

  • The ability to pass the screening required by Federal Human Services Office of Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding.

  • Must have a valid and current driver’s license and auto insurance.

  • ASL Fluency

Education and Experience • Bachelor’s degree in a behavioral science/related field and proof of 2000 hours of experience in the delivery of services to people with mental illness; OR • Master’s Degree in behavioral science or related field.

Preferred Qualifications for Targeted Case Manager II position: • Experience providing direct services to adults with serious and persistent mental illness

  • Experience providing Targeted Case Management and/or ARMHS

  • Knowledge of Recovery, Motivational Interviewing and Person-Centered approach
