Full job description
DMC Primary Care , a physician-owned, independent practice, with offices throughout southern New Hampshire, has been providing comprehensive care for entire families since 1964. We are seeking a full time, experienced Licensed Behavioral Health Clinicians (LICSW / LCMHC) to provide behavioral health services to Level 1 referred patients in an integrated, primary care setting.
The Behavioral Health Clinicians are considered a part of the primary care clinical team. Clinicians must be comfortable with:

Independently assessing individuals with behavioral health needs and/or substance misuse issues;

Developing treatment plans/interventions for individuals with mild to moderate mental health issues;

Interfacing with medication management professionals as needed;
-
Providing thorough communication to the primary care physician.
This opportunity offers some flexibility and may be able to include remote clinical work for a portion of the workweek after 90 days. At present there are no evening or weekend hours. Competitive compensation, medical benefits, and 401K are offered.
Position Qualifications:

Master’s in Social Work from an accredited college/university;

Currently Licensed in NH as an LICSW or LCMHC;

CPR certified;
-
Previous EHR experience preferred (including ability to document in electronic medical record);

Minimum 2 years’ mental health counseling experience in health care setting.
