Physical Therapist – $25,000 Sign-On Bonus

Your Role

As a Physical Therapist, you’ll deliver professional, evidence-based care in accordance with physician orders, state regulations, and agency policies. You’ll help clients regain independence and improve their quality of life through personalized rehabilitation plans.

Key Responsibilities

  • Conduct comprehensive assessments and develop individualized treatment plans.

  • Deliver therapeutic interventions, including manual therapy, modalities, and exercise programs.

  • Educate clients, families, and caregivers on therapy techniques and home programs.

  • Supervise Physical Therapist Assistants (PTAs) and Home Health Aides (as permitted).

  • Collaborate with interdisciplinary teams and participate in care conferences.

  • Monitor progress, adjust treatment plans, and communicate changes to physicians.

  • Maintain accurate, timely documentation in compliance with regulations.

Qualifications

  • Degree from an APTA-accredited Physical Therapy program

  • Active state license to practice as a Physical Therapist

  • 1+ year of experience or completion of a 6–8 week preceptorship

  • Home health experience and knowledge of Medicare Conditions of Participation preferred

  • Valid driver’s license and reliable transportation

  • Strong organizational and time-management skills

Physical & Mental Requirements

  • Ability to lift up to 35 lbs and assist with patient mobility

  • Frequent standing, walking, bending, and positioning

  • Adaptability to changing schedules and environments

  • Travel to client homes and Trinity Health locations required
