Employment Type: Full time Shift:

Description: Hospice Certified Nurse Assistant (CNA) /Home Health Aide

We have opportunities in the following areas:

  • Waterbury Region

  • Meriden Region

  • Hartford Region

Schedule: Full-time, Monday - Friday, 8am - 5pm, No Weekends or Holidays!

Must be a Certified Nurse Assistant in the state of Connecticut

Position Purpose:

Our hospice home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.

What You Will Do:

  • Follows care plan developed by RN.

  • Reports client changes in condition to RN, as described on care plan.

  • Assists clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.

  • Assists clients with household tasks including, planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.

  • Measures and records client’s oral, rectal and axillary temperatures, pulse respiration and blood pressure as ordered.

  • Performs range of motion and other simple procedures as an extension of therapy services as assigned.

  • Assists clients in self-administration of medication. This procedure must be assigned by an RN and documented in the clinical record. The Home Health / Hospice Aide shall not administer medication of any kind.

Minimum Qualifications:

  • High School diploma or GED required.

  • Active CT, CNA certification in good standing

  • Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.

  • Documentation of one year’s employment as an aide in an institution, home care, or hospice setting.

  • Must have current Driver’s license and reliable transportation to and from work site.
