Job Duties

The Southwestern Minnesota Opportunity Council, Inc. (SMOC) Health Services is looking to hire a Public Health Nurse (PHN) to meet the needs of our growth and program expansion to fully advance our Community Health and School-Based Health supported priorities of population-based care and health equity. This expansion reflects SMOC's commitment to whole-person care, improving access, and delivering preventative, responsive care across our clinics, community, and school-based settings.

The Public Health Nurse (PHN) functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in the state of Minnesota, and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct and the Minnesota Public Health Nurse Practice Counsel Guidelines.

SKILLS & ABILITIES:

-Valid unencumbered RN license in the state of Minnesota.

-Valid Minnesota Driver's License and willingness to travel between SMOC Health Services clinics and events.

-Maintain current CPR/BLS certification.

-Proficiency with electronic health record (EHR) systems.

-Strong communication and patient education skills.

-Ability to work both independently and collaboratively in a team environment.

-Strong organizational skills and attention to detail.

-Ability to lift 50 lbs. and withstand extended periods of standing and walking.

Minimum Education Required

Registered Nurse (RN); Bachelor (BSN) Degree in Nursing, and Public Health Nurse Certification in the State of Minnesota

Minimum Experience Required

Minimum of 2-4 years' experience in Public Health Nursing required.
