As part of a multidisciplinary team to deliver patient-centered nursing and rehabilitation services to patients. Support the Unit Manager to maintain excellent clinical standards and patient experience. Provides the highest level of service to all patient's, families, and visitors to ensure satisfaction. Responsible for ensuring all assigned residents receive all medications, treatments, and nursing care as ordered. Completes all required documentation as per state/federal guidelines.
Education, Knowledge, Skills and Abilities Required:

Completion of an approved LPN program.

Current with all Continuing Education (CE) requirements.

Perform clinical care professionally.

Serve as patient advocate and role model to other Team Members.

Deliver patient care with minimal disruptions and obstacles.

Ability to effectively work on a multidisciplinary team.

Excellent written and verbal communication skills.

Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.

Endurance to stand and walk for extended periods of time.

Ability to lift, push and pull a minimum of 50 pounds.
Education, Knowledge, Skills and Abilities Preferred:

Long-term care facility experience.

Prior work experience with geriatric and disabled populations.

Bi-lingual (Spanish).
Licenses and Certifications Required:
-
Licensed Practical Nurse.
-
BLS AHA Basic Health Care Life Support HCP Certification within 60 days of entering position.
Licenses and Certifications Preferred:
- IV Certification.
