Relevant Education Requirement: High school diploma or equivalent.

Required Certification/License: Must be a certified Home Health Aide (CHHA) in the state of New Jersey. Duties and Responsibilities:

Provide assistance with activities of daily living (ADLs) including bathing, grooming, dressing, and toileting.

Assist with mobility and transfer activities, as needed.

Support clients with medication reminders and monitoring.

Prepare and serve nutritious meals according to dietary restrictions and preferences.

Perform light housekeeping tasks such as laundry, vacuuming, and tidying up living spaces.

Offer companionship and emotional support to clients.

Communicate effectively with clients, their families, and healthcare professionals.

Document care provided and any changes in the client's condition accurately and timely. Reporting Structure: The Certified Home Health Aide reports to the Nurse Supervisor and Director of Nursing. They will collaborate closely with these professionals to ensure the highest standard of care for our clients. Additionally, they will receive guidance, training, and support from the nursing team to enhance their skills and knowledge in home care practices.
