Registered Nurse / Patient Care Manager

Location: Ormond Beach, Florida

Job Type: Full-Time

Schedule: Monday – Friday | 8:00 AM – 5:00 PM

Salary: $80,000 – $90,000 annually (based on experience)

Overview

A leading home health organization is seeking an experienced and compassionate Registered Nurse / Patient Care Manager to oversee clinical operations and support a dedicated team of healthcare professionals in the Ormond Beach area. This leadership role is responsible for ensuring the delivery of exceptional patient care, maintaining regulatory compliance, and fostering a culture of teamwork and clinical excellence.

The ideal candidate is a strong clinical leader with home health experience who is passionate about improving patient outcomes and supporting staff development.

Key Responsibilities

  • Provide clinical supervision and oversight to ensure patient care meets professional standards, agency policies, and regulatory requirements.

  • Coordinate, plan, and monitor patient care services to promote positive clinical outcomes.

  • Supervise and mentor clinical staff, ensuring consistent, high-quality service delivery.

  • Maintain effective communication with patients, families, referral partners, field clinicians, and office personnel.

  • Support quality improvement initiatives and departmental performance goals.

  • Promote a culture of professionalism, accountability, and service excellence.

  • Ensure compliance with Medicare guidelines and all applicable federal, state, and local regulations.

  • Maintain and promote workplace safety standards.

  • Encourage staff growth through coaching, education, and professional development opportunities.

Qualifications

  • Current Registered Nurse (RN) license in the state of Florida.

  • Home health experience required.

  • Two or more years of management or supervisory experience preferred.

  • Strong understanding of Medicare Conditions of Participation and home health regulations.

  • Excellent leadership, communication, organizational, and interpersonal skills.

  • $10,000 Sign-On Bonus

  • Competitive salary of $80,000 – $90,000 annually , based on experience

Pay Details: $80,000.00 to $90,000.00 per hour
