Social Work Counselor (FT)
Social Work Counselor
Location: Bohemia, NY | Schedule: Full time 35/hours | Schedule: Mon-Fri evening and weekend hours, as needed | Status: Non-Exempt | Rate: $22/hour
The Opportunity
Provide education and develop independent living skills in foster care children referred by the Department of Social Services in accordance with agency and regulatory requirements to ensure the highest quality of client care.
Responsibilities

Conduct monthly home visits.

Assist in developing and facilitating Independent Living Skills Workshops
-
Complete a Step Up Independent Living assessment and treatment plan in partnership with client, foster family and caseworker.

Complete and maintain client case records in a timely manner and in compliance with agency and other regulatory standards.

Complete all required reports, correspondence and other paperwork and submit by requested deadlines.

Coordinate service efforts with DSS caseworker regarding client's plan

Task supervisor for student intern
-
Provide contacts and supports for the foster families to facilitate the teaching of independent living skills in the home.

Provide aftercare services to all PI program youth
-
Recruit, assess and evaluate potential scholarship recipients

Ability to work remotely

Attend conferences, trainings and workshops as appropriate

Assume other tasks and responsibilities assigned to support the operations of the program
Qualifications
-
Education: Bachelors degree in Human Service
-
Certificates or Licenses: NYS drivers licenses
-
Experience: One or more years of experience working with adolescents
