Full job description

Community Health Worker (Bilingual Spanish) Must live on Staten Island, NY | Monday–Friday, 8:00 AM–5:00 PM | Direct Hire

Join a mission-driven nonprofit dedicated to improving access to healthcare, housing, and food for underserved communities throughout Staten Island. This is an excellent opportunity for someone passionate about community outreach, public health, and helping others. Entry level candidates are encouraged to apply!

Compensation:

  • $50,000–$55,000/year
  • Annual salary increases

What You'll Do:

  • Conduct community outreach and health screenings
  • Support Medicaid members and individuals with asthma or chronic health conditions
  • Complete health assessments and questionnaires
  • Serve on home-visit teams of 2 (approximately 8 per month; some virtual)
  • Connect community members with resources and health-related services
  • Assist with environmental health initiatives and supply coordination
  • Present educational workshops and community programs
  • Maintain accurate records across multiple systems

Requirements:

  • Fluent in Spanish and English (Required)
  • Must live in Staten Island, NY or within a few mile radius.
  • Passion for community health and helping underserved populations
  • Ability to work independently and stay organized
  • Comfortable with outreach, home visits, and public presentations
  • Tech-savvy and able to learn multiple systems quickly
  • Healthcare, social services, community outreach, or customer-facing experience preferred
