Position: Home Health Physical Therapist (PT)

Location: Elizabethtown, PA

Coverage Area: Dauphin County, PA

Schedule: Monday–Friday | 8:00 AM – 5:00 PM

Employment Type: Direct Hire

Salary: $108,000 - $132,000 annually

$20,000 Sign-On Bonus!

What You'll Do

As a Home Health Physical Therapist, you'll make a meaningful difference every day by helping patients safely recover in the comfort of their own homes.

Responsibilities include:

Perform comprehensive physical therapy evaluations

Develop individualized, evidence-based treatment plans

Deliver one-on-one therapeutic interventions to improve strength, balance, mobility, and functional independence

Educate patients and caregivers on home exercise programs and injury prevention

Monitor patient progress and modify treatment plans as needed

Collaborate with physicians, nurses, and interdisciplinary care teams to achieve exceptional patient outcomes

Maintain accurate clinical documentation and ensure regulatory compliance

Qualifications

• Current Pennsylvania Physical Therapist license

• Master's Degree in Physical Therapy from a CAPTE-accredited program

• Valid CPR Certification

• Valid driver's license with the ability to travel throughout the assigned coverage area

• Home Health or Community-Based Physical Therapy experience preferred

• Strong clinical assessment, communication, and patient education skills

Salary: $98,000 - $120,000 annually

Minimum of 9 months of Physical Therapy experience required.

Pay Details: $98,000.00 to $132,000.00 per year
