What You'll Do & Key Responsibilities:

Assess patients’ psychosocial needs and complete social histories

Document care plans, progress, and follow-up per regulations

Provide counseling and emotional support to residents and families

Support patients and families with adjustment, rights, and orientation

Coordinate referrals and act as a liaison with community resources
-
Partner with the care team to enhance quality of life and outcomes

Plan and support patient, family, and community engagement activities
-
Promote facility programs and services to referral partners
What it takes to be a Social Worker at Care Initiatives Hospice:

Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education and current, valid social work license (LMSW).

Hospice or palliative care experience, preferred.

Two (2) years of verifiable experience working as a Social Worker, preferred.

Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
