Job Description:

  • Conduct comprehensive patient assessments to gather data on physical, emotional, and social health status.

  • Develop and implement individualized care plans based on assessment findings, setting goals and outlining interventions.

  • Administer medications and treatments accurately as prescribed, ensuring patient safety and adherence to protocols.

  • Monitor and evaluate patient responses to interventions, adjusting care plans as necessary and documenting progress in the electronic medical record.

  • Collaborate with multidisciplinary teams to resolve patient care issues and ensure coordinated, comprehensive care.

  • Educate patients and families about health conditions, treatment plans, and self-care strategies to empower them in managing their health.

  • Supervise and guide nursing personnel in delivering safe, therapeutic, and quality patient care, maintaining a professional rapport with all team members.

  • Ensure safe and effective handoff communications by using standardized communication tools during shift changes and patient transfers.

  • Perform and oversee complex patient care interventions while maintaining aseptic technique and infection control practices.

  • Provide discharge planning to ensure a smooth transition from hospital to home or another care setting, addressing the patient's needs and resources.

  • Engage in ongoing education and professional development to maintain clinical competencies and improve patient care.
