Heart Failure (CHF) Cardiologist – Non-Interventionalist MD Boulder Community Health, Boulder, CO 80303

$798,000 annually

Boulder Community Health is seeking a Board-Certified/Board-Eligible Heart Failure (CHF) Cardiologist (Non-Interventionalist) to join our established and collaborative cardiology team. This role focuses on the comprehensive management of patients with advanced heart failure, working closely with a multidisciplinary team to deliver high-quality, patient-centered care across the continuum.

Qualifications • MD or DO from an accredited medical school • Board Certified or Board Eligible in Cardiology • Fellowship training in Heart Failure/Advanced Heart Failure preferred • Active or eligible for Colorado medical licensure • DEA certification required • Demonstrated experience in managing complex heart failure patients • Strong commitment to patient-centered care and multidisciplinary collaboration

Responsibilities • Provide outpatient and inpatient care for patients with congestive heart failure (CHF) • Manage advanced heart failure therapies, including guideline-directed medical therapy (GDMT) • Collaborate with cardiology, hospitalist, and specialty teams to coordinate comprehensive care plans • Participate in the development and growth of heart failure programs and services • Support transitions of care, including discharge planning and outpatient follow-up • Engage in quality improvement initiatives and evidence-based practice implementation • Provide patient and family education regarding disease management and treatment options • Participate in call rotation as part of the cardiology team
