Full job description

S ummary

In partnership with the Lead Community Outreach Dietitian, plans, organizes, implements, and promotes the produce prescription program, Veggie Rx to Heal. The VeggieRx to Heal program comes to the aid of our communities most vulnerable patients at risk for diet-related disease and food insecurity. Helps develop processes, procedures, and carries out grant work from start to finish, during the identified grant period. When applicable, works directly with clinicians and staff to introduce the produce prescription program and the process for referring patients into VeggieRx to Heal. When applicable, obtains clinician referrals from Electronic Medical Record system (EMR), enrolls patients into VeggieRx to Heal program and monitors participant engagement. Works with Office of Sponsored Projects and Mirro Research team to ensure all grant expenditures, activities and data are collected and reported as per the federal guidelines. Works with external partners to coordinate market and grocery store redemption sites.

Education

Bachelor's degree in nutrition, public health, community relations, or other related field.

Licensure/Certification

Project Management Professional (PMP) certification preferred.

Experience

Prior project management experience and or grant management experience preferred. Mus t have solid experience with Excel and have the capability to learn and utilize other data platforms, such as Smart sheets.
