Position: Blended Service Coordinator

Hourly Rate: $22.50-$27.84

Schedule: Monday through Friday 9a-5p, with some weekend on-call availability

Location: Greater Pittsburgh Area

Service Coordinators play a critical role in fulfilling the mission of TCV by providing professional assessment, coordination of mental health services and supports, service planning, linkage, referral, and advocacy services required for a safe and healthy community life based on the individual’s level of needs. The incumbent works as a member of a treatment/service team in order to promote individuals’ mental health recovery. We are looking for people who believe in a strengths-based approach and use positive practices to help guide their everyday activities. The Service Coordinator position is designed to:

  • Advocate for and with the individual being served to ensure responsiveness from natural, community, geriatric and specialized services/supports. Advocacy includes providing information, removing barriers, creating options and resolving problems.

  • Maintain an accurate and timely record of Service Coordination activity by completing service documents within 24 hours. Reviewing charts for compliance with regulations and preparing for formal audits.

  • Ensure that individuals being served needs are met through the utilization of natural supports (family, friends), community and geriatric services and specialized services (MH/MR, supported employment, OVR, D&A). Assists individual and family to identify, link, access and coordinate such resources.

  • Provide on-site support for all in-patient MH commitments by ensuring all alternatives to hospitalization have been considered first then advocating for admission.

  • Provide culturally competent services with consideration for the individual's racial, religious, sex, sexual orientation, age and ethnic background and identification.

  • Attend training programs provided through the State, County and TCV to assure that the incumbent is up to date on new approaches, best practices and recovery-oriented services. (37.5 hours of annual training is required.)

  • Ensure the frequency of contact with the individual being served is based on the individual’s needs, but at the very least once a month. Provide a full range of Service Coordination services to a designated caseload.

Qualifications

EDUCATION & EXPERIENCE :

Qualifications:

  • Bachelor’s Degree with major course work (min. 12 credit hours) in Social Work, Psychology, Criminal Justice, Pastoral Counseling, Counseling Psychology, Rehabilitation Counseling, Sociology, Education with a certificate in Special Education or Nursing with a Registered Nurse License. All degrees must come from an accredited college or university.

  • Minimum of 1 year of experience in human services, community based preferred, and working knowledge of human services paperwork and regulation guidelines.

  • Act 33/34 (PA State Criminal and Child Abuse) Clearances and FBI Clearance.

  • Valid PA driver’s license and driving history of three (3) years with MVR required.
