Full job description
Employment Type: Full time Shift: Rotating Shift
Description: Position Purpose: Our inpatient Behavioral Health Care units offer evaluation and treatment to adult, teen and adolescent patients who are in an acute phase of psychiatric illness or are experiencing an emotional or life crisis. We individualize each treatment program, led by a psychiatrist who directs a professional health care team in a multidisciplinary team approach. What you will Do:
- Conducts initial and ongoing observations to gather data.
- Assists with completion of initial admission database.
- Collects patient data and completes ongoing observations as directed by the RN and/or unit specific standards.
- Assists in development of the outcome/goal-based plan of care.
- Collaborates with RN and interdisciplinary team to identify patient problems, outcomes/goals and nursing interventions.
- Assists RN in identifying appropriate referrals (i.e., Patient/Family Services, Dietary, and Spiritual Care) and education needs.
- Implements the plan of care including procedures, treatments, physician orders, nursing orders, therapy orders, education plan and medications.
- Prioritizes and works effectively within the team.
- Participate in expected outcomes/goals for each patient and assists team to achieve.
Hours/Schedule:
.8 fte, night shift, 11pm-7am
8 hour shifts during the week
Minimum Qualifications: Current IA LPN No experience required
