Job Description:

  • Maintains a safe and clean working environment by complying with policies, procedures, rules, and regulations.

  • Develops and maintains positive working relationships within the practice and other departments. Other duties as assigned.

  • Implements and monitors patient care plans to ensure effective treatment. Monitors, records, and communicates patient condition as appropriate.

  • Serves as the primary coordinator of all disciplines for well-coordinated patient care.

  • Notes and carries out physician and nursing orders accurately and promptly. Assesses and coordinates patient's discharge planning needs with members of the healthcare team.

  • Leads and mentors' staff to ensure optimal performance and competency in care delivery.

  • Conducts holistic, individualized assessments of each patient considering age, physiological, and psychological status.

  • Documents all phases of patient status while responsible for their care. Participates in the transfer of patient information to ensure continued quality care. Education:

  • Associate's of Nursing [Required]

  • Bachelor's of Nursing [Preferred]

Licenses and Certifications:

  • Registered Nurse (RN) [Required]
