Location: Morgan Hill, CA

Pay Rate: $40/hour

Schedule: Full-Time | Sunday - Thursday | Night Shift (NOC)

Contract Length: 13 Weeks

Are you a compassionate and clinically skilled Licensed Practical Nurse looking for an opportunity to make a difference in the lives of long-term care residents? We are seeking a dedicated LPN - Long Term Care & Behavioral Health Support professional to join our team in Morgan Hill, California.

In this role, you will provide high-quality nursing care, behavioral support, and clinical oversight for residents in a long-term care environment. You'll work alongside an interdisciplinary team to help ensure residents receive the personalized care, treatment, and support they deserve.

What You'll Do

• Deliver compassionate nursing care within your scope of practice

• Monitor resident health status and make sound clinical judgments under appropriate supervision

• Provide behavioral support and direct resident care

• Implement and support individualized treatment and behavior plans

• Participate in interdisciplinary care planning and team collaboration

• Supervise DSPs and CNAs when serving as lead staff

• Deliver structured therapeutic programming and resident support throughout assigned shifts

• Maintain accurate and timely documentation

• Follow established clinical protocols and nursing standards

• Report resident concerns in accordance with mandated reporter guidelines

• Support positive resident outcomes through effective communication and patient advocacy

What We're Looking For

Required Qualifications

• Current Licensed Practical Nurse (LPN) license in the state of practice

• Current BLS certification

• Current Health Certificate (if applicable)

• Current PPD or Chest X-Ray documentation

• Successful completion of required clinical specialty screening assessments with a score of 80% or higher

• Successful completion of background screening requirements

Preferred Qualifications

• Minimum of one year of professional nursing experience

• Experience in Long Term Care, Assisted Living, Behavioral Health, Psychiatric Care, or related settings

• Strong critical thinking, assessment, and communication skills

• Ability to effectively manage multiple priorities in a fast-paced environment

The Long Term Care LPN assumes responsibility and accountability for the delivery of care for patients in a Long Term Care/Assisted Living Facility. The nurse demonstrates ability to make clinical judgments in an effective and efficient manner with supervision, as well as critical thinking and performance ability in the coordination of patient care. Additionally, this individual performs according to nursing standards and is accountable in managing patient care and assisting others in the management of patient care.

Minimum Requirements:

• Current Licensed Practical Nurse License for the state in which the nurse practices

• Current Health Certificate (if applicable)

• Current PPD or Chest X-Ray

• One year prior professional nursing experience preferred

• Current BLS card

• Successful completion of screening tests for the clinical specialty area(s) in which the LPN practices with a score of 80% or greater

• Successful completion of Background Screening Process
