Job Description

Position Overview:

A growing home health agency in Michigan is seeking an Occupational Therapist (OT) to join their team. This individual will play a key role in supporting home health patients and providing supervision to an existing Certified Occupational Therapy Assistant (COTA). The organization is actively expanding its therapy services and is looking for an OT who can help support patient care and allow the agency to accept additional referrals.

Responsibilities:

Provide skilled occupational therapy services to patients in their homes

Complete routine follow-up visits and reassessments

Perform supervisory visits for the Certified Occupational Therapy Assistant (COTA)

Develop and oversee patient treatment plans

Collaborate with interdisciplinary care teams to ensure quality patient outcomes

Maintain accurate clinical documentation and comply with home health regulations

Participate in discharge planning and patient education as needed

Patient Volume:

Current caseload is approximately 14 patients per week

Primarily routine visits

No Start of Care (SOC) responsibilities at this time

Program is continuing to grow, allowing for future volume increases

Skills and Requirements

Active Occupational Therapist (OT) license in the state of Michigan

Home health experience preferred

Experience working independently in a community-based setting

Strong communication and patient care skills

Ability to supervise and collaborate with a COTA Previous experience supervising therapy assistants

Medicare home health experience

OASIS knowledge
