- Pay Range: $85,000 - $95,000 Ready for more flexibility and one on one patient care that actually makes a difference
Phoenix Home Care and Hospice is hiring an Occupational Therapist to support patients in the Jefferson City and Columbia areas. This is your chance to move beyond the clinic setting and help patients regain independence in their own homes.

What You Will Do

Provide occupational therapy services in the home based on the physician plan of care

Evaluate patient needs and develop individualized treatment plans

Focus on improving daily living skills, safety, and independence

Collaborate with the care team to ensure quality outcomes

Educate patients and families on techniques and care plans

Maintain accurate and timely documentation

Follow all safety and compliance standards What You Need

Active Occupational Therapist license in Missouri

Home health experience preferred but not required

Strong clinical judgment and time management skills
